• Please submit the medical and permission forms as soon as possible.
• Medical Forms are required by August 1st for Fall enrollees. Our nurse needs lead time for processing and follow up.
• All other forms, summer transcripts etc. are requested by August 1st

• New U.S. and international students need to submit the new student forms.
• Returning students need to submit all the returning student forms (these need to be updated each year).
